Office Policies

We believe that all of our patients deserve our undivided attention when in our office. We do our best to be very thorough while at the same time working as efficiently as possible. While we may sometimes encounter unforeseen situations which may cause a delay, we try to manage them as best we can. In an attempt to help us be timely, we ask that our patients come 10 – 15 minutes before their appointment time to allow for accurate check in.

There are Medical History and Patient Information forms available on this website… you can download them and complete them before you come in!

Our doctors and staff at Gontarek Eye Care, Optometrists, LLC know that YOU are the reason we are here. We appreciate your feedback…both positive and negative. Please share your comments and concerns with us! It helps us to serve you better!

Cancellations

When patients miss appointments, it affects our efficiency. Therefore, any patient that either fails to cancel with at least 24 hours notice or misses an appointment, for a second time, will be charged a $25 fee. If the missed appointment occurs on a Saturday, the fee is $50.

New Patients

Please let us know if someone recommended our office to you! We realize that word of mouth is vital to the success of our practice. We like to thank our patients who refer others to us. With our ‘V-Eye-P’ Reward Program, patients can earn vouchers to use towards products and services at our office.

We know that we ask a lot of questions of our new patients… demographic information, medical history, insurance information, and lifestyle and hobby information. We do this so that we may address your needs, so we can take care of your eyes, and most of all, to make sure all of Your questions are answered during your time in our office. Please bear with us as we gather the information we need to provide the best possible care for you. When you come in for your appointment, we ask that you please have all insurance cards, picture ID, and information about any medication you may take. If you have glasses, bring them all with you. If you wear contacts, bring boxes or blister packs with you.

Prescriptions and Refills

Prescriptions for eyeglasses, contact lenses, and certain medications needed in the treatment of eye conditions can be written only by our eye doctors. You must be seen once a year in order to update your prescription for glasses or contacts. Medications for eye conditions are prescribed on an as needed basis. If you call our office for refills on medications, the doctor will need to authorize those refills. The individual situation will determine how often you must be seen by the doctor in order to obtain refills on medications.
